Maternity Package

• Prenatal, birth and postpartum care with a Licensed Midwife, midwife assistant and possible students. Includes:

  • Fetal heart monitoring, position checks, and uterine growth monitoring

  • Maternal vitals and well-being exams

  • Laboratory procedures -ie routine prenatal panels, glucose, and GBS screening below)

  • Nutritional, exercise, mindfulness, and prenatal education counseling

• Your choice of home birth (travel within 50 miles) or birth center

• Access to midwife via phone, text or email during regular business hours and after-hours for emergency

• Filing of official state birth certificate and application for Social Security number

• Newborn hearing screening

• Newborn metabolic screening procedure

• Newborn CCHD screening (heart defect screening)

• Call for pricing and insurance questions

Payment Details

I accept payment through Paypal, Cash, Check, Venmo, HSA, Flex, and Credit Card. Some insurances accepted.

• Early Pay discount if paid in full by 20 Weeks

• Late entry discount if coming into care after 32 weeks with proof of adequate prenatal care

• Additional mileage fee outside of 50 miles from Payson, Utah (ask for a specific quote)

• Repeat client discount $$

• $500 Non-refundable deposit holds your place in my calendar.

Additional Fees

• Rhogam injections for prevention of Rh isoimmunization … $150 each (Only about 10% of people need this. Please ask for details if you are unsure.)*

• Ultrasound Fees … Additional ultrasounds as desired or needed as charged by the ultrasound technician.

• Additional labs beyond standard labs, if needed

• IV therapies during pregnancy

• Placenta encapsulation (includes pick up & drop off) … $160 ​(optional)
